Division of Geriatric Medicine
The Scripps Clinic Medical Group Division of Geriatric Medicine provides care for patients in the post-acute Skilled Nursing Facility (SNF) setting, serving as a link between Scripps hospitals and primary care providers and the independently owned and operated nursing facilities. The complexity of the post-acute patient continues to increase, and Scripps Clinic Geriatricians aim to optimize our patient’s quality of life by considering the whole patient, including medical, psychological, functional, and social needs. The Scripps Clinic team works closely with patients and families to develop goals of care that are realistic and achievable, and to facilitate early development and communication of appropriate discharge plans. Patients are provided with direct access to provider’s contact information to improve accessibility. Care is documented in the Epic electronic medical record to improve coordination with other providers in the Scripps system. The division leads system-wide quality improvement projects targeted at improving transitions of care from the hospital to the SNF, and from SNF to home. Scripps Clinic Geriatric Medicine does not currently offer an office-based practice.
